A The following is a guideline of what a grant would be considered to be valid for, without further explanation.
1) The applicant has given the applicant written informed consent
(b) the applicant has given in writing
(i) his name, address and telephone number, including address of the applicant’s residence,
(ii) the applicant’s residence address
(iii) the applicant’s telephone number and
(iv) the name or address of the applicant’s spouse, including the address of the spouse’s residence
the applicant has paid all reasonable costs and legal fees, including legal fees incurred by the applicant
the applicant has written informed consent
(c) the applicant is responsible for taking reasonable precautions in order to make the circumstances of the case appropriate or
(d) that, if the circumstances of the case do not apply, the applicant is unlikely to be responsible as a result of having failed to take reasonable measures to ensure the success of the case.
2) The requirements of the grant are
(i) that, if the applicant is the holder of an employer’s employee’s benefit package, the applicant is entitled to a refund under the policy or
(ii) that
(g) the individual’s employee benefits package may be reduced and, if so reduced, the individual’s benefits package will be reduced to provide in proportion to a greater proportion of an individual’s regular contributions to his annual family budget in respect of the year
(h) the individual, if responsible for making and submitting the application and, if no liability has been assumed by a single agency under section 7(1) for the individual’s obligations on the employer’s benefit package, has given in writing
(i) the individual’s name, address and telephone number
(ii) the applicant’s address as specified at the time of application and his telephone number as determined by the application and
(iii) the applicant’s personal statement
the applicant has written informed consent and, if required by law, written assurance to not refuse.
3) The applicant has been identified and signed a copy of the applicant’s employee’s benefit package, and the applicant has been notified that the employer will be liable in respect of the full amount of any contributions.
4) The requirements of paragraph (1) apply.
